چکیده
They have emphasised the role of hypertension as a risk factor. They have ascribed it to Diabetes if microalbuminuria is persistent on 2 occasions tested at 8 months interval. It is important to do so as at this stage a proper treatment can arrest further progression. Renal biopsy at this stage may not be rewarding for its therapy affectivity. In my own Editorial on this article, I had alluded to the Fearless forecasts of JNC VIII by George Bakris; as a personal communication I had, where we felt that time had come to combine Stages of Diabetes, CKD and Hypertension per se for proper understanding of the impact all 3 on a given patient. This would lead to much better understanding on the part of the physician. Moreover presence of microalbuminuria heralds increased Cardiovascular morbidity in such patient. Eye is certainly window to the body organs esp. to the Kidneys and High blood pressure. This certainty forms a part and panel of full clinical examination of every Diabetic patient. I must also however state that both kidneys Biopsy changes and eye changes can lag behind early detected microalbuminuria. If prevention is the aim; both detection of microalbuminuria and early eye changes detected by ophthalmologist are a must.

متن کاملPulmonary Paragonimiasis Mimicking Tuberculosis.
Paragonimiasis is a disease which is frequently misdiagnosed as pulmonary tuberculosis. Proper epidemiological data are lacking from this part of the country on zoonotic parasitic diseases, and newer diseases are emerging in the current scenario. Systemic manifestation of such diseases as cysticercosis, paragonimiasis, hydatidosis, and toxoplasmosis are fairly common. The incidence of acquired t...

Clinical manifestations of lung involvement could be acute: asthma –like syndrome, or Loeffler’s syndrome, with dyspnea, wheezing, cough; or chronic such as hemoptysis or right heart failure signs. Acute...
